We as a community and world wide continue to learn together how to mitigate the spread of COVID-19. At Fifth Avenue school we encourage our students and families to continue following CDC guidelines. Please continue to remind and encourage your children to practice proper mask etiquette in school by using a well-fitted mask that covers the nose and mouth. Encourage your child to continue practicing social distance protocols and proper hand washing after touching the mask or before touching the face or eating.

 

As a reminder parents and guardians are required to wear a mask when entering the school building regardless of vaccine status.

 

A student who appears ill should remain at home. All students who become sick while at school or experience issues that require a doctor's evaluation will be sent home. Therefore it’s very important to provide up to date emergency contact information to the health office as well as to the district office. 

 A student's health is pivotal to successful learning.

MEDICATIONS:

All medications, prescription or over the counter, (which includes throat lozenges - herbal or natural):

1. MUST be prescribed by a licensed physician, and the written authorization brought to the health office.

2. MUST have written authorization by the parent/guardian to allow the nurse to administer the medicine.

3. MUST be in the original pharmacy container with the correct label and dosage.

4. MUST be delivered or picked up by the parent/guardian or an adult.

CHILDREN ARE NEVER TO TRANSPORT MEDICATION TO OR FROM SCHOOL.

The potential for severe harm always exists with any type of medication, so these procedures are in place for your child’s safety.

INJURIES

Children with fractures, stitches or other serious conditions will need a doctor’s note to be excused from physical education. NO PHYSICAL EDUCATION ALSO MEANS NO RECESS. Written medical clearance from a doctor is necessary to return to full activity. Home injuries should be cared for at home, since they did not occur in school.

ABSENCES

If your child is absent please call the school by 9:30 a.m. A written note from a parent or physician is required when the child returns to school. If an absence is due to illness, children must be without symptoms and fever free for at least 24 hours before returning to school. Children who are absent from school will not be allowed to return to school on that day for parties, school pictures, or after school programs.

PHYSICALS

All new students and students entering grades Pre-K or K and 1 are required to have a physical examination. It is preferred that it is done by your family physician, if not; the school physician will examine your child in school.
