Student Registration

 

STUDENT REGISTRATION

 

Parents who reside within the boundaries of the Íæż½ã½ã School Íæż½ã½ã must register all school-age children at the OFFICE OF STUDENT SERVICES & CENTRAL REGISTRATION.

Registration is by appointment, only. Please call 968-2554 for details.

Unless otherwise indicated or not applicable, the following documents are required at the time of registration:

Documents for Students

  • Original birth certificate or other official document deemed sufficient to establish age and parentage.
  • Certificate of immunizations.
  • Social Security card. (optional)
  • Latest transcript, attendance and discipline records (Grades 6 through 12.)
  • Latest report card (Grades 1 through 8.)
  • Custody and/or guardianship documents (divorced/separated parents or other special circumstances or restrictions.)
  • Marriage certificate - to designate step-parents as guardians.
  • Individualized Education Program/Plan or I.E.P. (Special Education students, only.)
  • Form DSS - 2999 (foster children, only.)

Proof of Residency from Parent/Guardian

  • Primary Proof of Residency:

    • Home Owner: deed, current mortgage statement or tax bill
    • Renter: Notarized Lease (Original) or Landlord Affidavit (Form supplied by the Íæż½ã½ã Schools), and
  • Supporting Proof of Residency Documentation:
    • Current utility bill (cable bill, LIPA, fuel, water) or
    • Current checking, savings or major credit card account statement.
    • Other official documents or circumstances deemed sufficient to establish residency.

*Note: telephone bills are not acceptable.

 

Photo Identification of Parent/Guardian

  • NYS Drivers License
  • NYS Identification Card
  • Military Identification
  • Passport
  • Resident Alien Card
  • Other official identification deemed sufficient to establish identity.

PLEASE NOTE: PARENT(S) AND STUDENT(S) MAY ALSO BE REQUIRED TO MEET WITH SCHOOL BUILDING PERSONNEL (GUIDANCE COUNSELOR OR SCREENING TEACHER) BEFORE STUDENT BEGINS CLASSES.
